You and your partner are both requested to attend the Initial Consultation,
which will last approximately ½ hour. You will both need to fill out medical
history forms, which can be mailed to you or accessed and downloaded
from this website. Please call our office (631-444-4686) for instructions.
You should request copies of your medical records from the physicians who
have previously been treating you and your partner. Record release authorizations
are also available on this site. Include the following test results, if they
have been performed:
- Results of all hormonal tests performed during the past year.
- Copies of operative notes of any pelvic surgery, including diagnostic laparoscopy.
- Reports of recent semen analyses and male infertility diagnostic tests performed during the past year.
- Hyterosalpingogram (HSG) written reports. We would also appreciate your bringing copies of the actual films from the radiologist, if this is possible.
- Stimulation sheets from previous in vitro fertilization (IVF) or intrauterine insemination (IUI) treatment cycles
- Copies of embryology lab reports, if you have undergone prior IVF cycles

Our Center participates in a number of insurance plans. We believe that it is important that you understand your benefits prior to initiating an evaluation and treatment. This information will help you to make treatment choices as they are presented to you by your doctor. Your insurance may be limited to a specific dollar amount or a specific number of treatment cycles. If you lack further coverage for infertility treatment, we do offer no-interest deferred payment plans. We believe that understanding insurance benefits and obtaining the necessary authorizations before treatment can be confusing.
